Reproducible on OpenBSD, mixer 4.6.1, Xfce 4.8.
Hmm, After open & close the mixer plugin's property dialog, it works fine on my system. Adjusting volume by mouse wheel is available and tooltip appears now.

I cannot confirm this. The issue persists on my system. Maybe it's an issue with configuration files being different on both of our systems though. However I could not locate those config files for the mixer plug-in.
As of today, I gave up on waiting for this bug to be fixed and installed a nice little app that does what I want. It's in AUR for those of us who use Arch. It's called "volumeicon": http://aur.archlinux.org/packages.php?ID=35793
