It seems the renamer is unable to rename a file if i want to replace something in both name and suffix.

It displys the preview correctly but wont enable the rename button

Cannot reproduce, neither with 1.6.15, nor with 1.8.0

Would it be possible for you to compress and upload the related folder ?

could the reason for this that this would create a file conflict? if so then it should be noted or the renamer should ask if i want to rename/replace the files with the same names

Yes, this pretty much looks like the reason for the trouble ;)

So I quess we should add a better way to show that there are conflicts.

This bug reminds of Bug #14229.
If we were to make some improvement, I can only think of:
1) Display an alert dialog, I don't like it, too much intrusive.
2) Scroll to the first occurrence of conflict, I'm not sure this is enough for some people to understand what's wrong.
3) Add GtkIconBar to Renamer dialog and show it in case of conflict.

2 & 3 are good enough IMHO.
Since this is an improvement, we'll eventually introduce it in 1.8.x, not in 1.6.x.
