Now finally I have found a test case to reproduce:

Pre-requisites:
===============
You need to have shortcuts defined for custom actions assigned to directories (e.g. open terminal here). I have not tested if custom actions assigned to files are affected by the same problem.

Steps to reproduce:
===================
1) Open a thunar window.
2) Look at menu File, ensure the custom actions are listed with their shortcuts there.
3) Create a directory, e.g. with context menu->New->Folder.
4) Look at menu File, behold the custom action shortcuts are gone!

Additional information:
=======================
The shortcuts are not lost permanently. If you open another window, the shortcuts will appear there.

Also, the same thing will happen if one deletes an existing folder. Shortcuts gone.

Opening the right click context menu restores the shortcuts.
